Translation to Practice® Program
AAFP members attending a chapter meeting may engage in one t2p™ exercise per CME activity. Two CME activites at the Fall Conference will be eligible for t2p™ credit.
“t2p™” stands for Translation to Practice®. t2p™ complements CME activities already approved for AAFP Prescribed credit. It offers learners the opportunity to apply what they learned in the CME activity, and report back on how the learning was translated into practice. Translation to Practice® began as a pilot in 2010, and has been administered by AAFP and many participating chapters since that time. To learn more about t2p™ visit the FAQs here.
The material presented in all sessions is offered solely for educational purposes. NAFP disclaims any and all claims that may result from use of information presented in this activity. Material contained herein may be graphic but is intended for clinical use.
It is the policy of NAFP that all CME planning committee members, collaborators, speakers, and NAFP staff disclose relationships with commercial entities. Disclosure documents are reviewed for potential conflicts of interest and if a conflict is identified, it is resolved prior to confirmation of participation. Only those participants who have no conflict of interest or who agree to an identified resolution process prior to their participation were involved in this CME activity.
